MyChart
Stay connected to your healthcare with MyChart through this secure online patient portal. You can manage appointments, access test results, request medication refills and pay bills — for yourself and your approved appointees. Brookwood Baptist Health MyChart is your healthcare connection, whenever you need it and wherever you are.
- MyChart provides secure access to your health information from your hospital visits at Citizens, Princeton, Shelby and Walker Baptist Medical Centers.
- You will receive an activation code from your provider or with your discharge instructions to allow you to create and access your MyChart account.
- You will need is a valid email address to sign up. Use your phone, tablet or computer to access your health information.
- To protect your privacy, a personal login is required at sign-in.

Access the MyChart Portal
With My Health Rec, you can:
- View some pathology, radiology and lab results
- Access health records, including immunizations, medication list and known allergies
- View hospital records, such as discharge summaries and notes
- Download health records to share with other providers
- Pay medical bills online
The Apple Health Data app for iPhone users offers additional ways to access your health information.
For some results/records that are not able to be accessed online, please contact the respective hospital Medical Records department.
How to Get Started
You can sign up for your My Health Rec patient portal account at your next inpatient visit or by using our self-enrollment tool. My Health Rec is available for all patients who have registered and had services provided at Brookwood Baptist Medical Center.
