You can access your inpatient medical information online through our patient portal.
Request a hard copy of your medical records
We protect your health care information in compliance with federal and state requirements. If you’d like a copy of your medical records, please download the Release of Information form for your hospital listed below, then print, complete, sign and mail (or fax) the form to the hospital.

When you come to pick up your medical records, please bring photo ID. If someone other than the patient is picking up the records, that person needs photo ID and an original signed authorization letter from the patient. There may be a charge for creating a copy of your records.
